Registration Information
Before completing the registration form, please read the following information which is designed to help you complete your registration.

Registration Fees:
     Registrant for a single attendee $ 400.00
     Registrant with spouse $ 650.00

A guest ticket for individual events may be
purchased by a registrant. Ticket prices are:

     Reception & Dinner $ 160.00
     Breakfast Buffett $ 55.00
     Lunch $ 100.00

Registration Methods :
The registration form MUST be accompanied by payment in order to be processed and to ensure advanced registration. You may register:

Online - By completing the online registration
form at http://www.txoga.org/lonestarforum, with your Visa, MasterCard or American Express credit cards or

Mail - By printing and completing the online
registration form & sending it to: TXOGA, 304 W. 13th Street, Austin, TX., 78701-1823 or

Fax - By using the 24 hour FAX (512) 472-3859 to register with credit cards. If using FAX, do not mail the form, or

Phone - By calling (512) 478-6631.

 

Golf:
Golf will be played in foursomes with tee times
beginning at 1:00 pm on Sunday, October 12th. You may request to play in a in specific foursome. Please so note on the registration form. Beverages will be available during play.

Special Needs:
Individuals with special needs (mobility, access, etc.) should indicate those needs on the registration form.

Meeting Attire :
Business casual is appropriate for all meetings and social functions.

Refund Policy:
Cancellations received before October 3rd, entitle the registrant to a full refund. Cancellations received after October 3rd, entitle the registrant to a 50% refund. Refunds
will not be made if refund request is received after October 8th.
Refunds will not be made for no-shows.

On-Site Registration:
If you have not registered for the meeting by October 8th, please plan to register on-site at the Four Seasons Resort and Club - Las Colinas in Dallas.

Hotel Information
A block of rooms at a discounted rate has been reserved for forum attendees at the Four Seasons Resort and Club - Las Colinas in Dallas. To make reservations, call the hotel directly at (972) 717-2499. Be sure to tell them you are
with the Lone Star Forum to receive the discounted rates:

     Superior...........$245.00
     Deluxe.............$285.00
     Villa.................$300.00
